By signing this form, I give permission for my self/child/ward to be video recorded, audio recorded and/or photographed. I understand that Loving Guidance, LLC/dba Conscious Discipline (Hereinafter referred to as Conscious Discipline) of Sanford, Florida shall have total and sole ownership of the video, audio, photograph or other media, including the right to edit, broadcast, reproduce and to offer for sale to the public. Conscious Discipline shall own all rights to copyright and the right to license others to use the video, audio, photograph or other media. This consent and release is intended by me to be used by Conscious Discipline forever and throughout the world and for whatever purposes Conscious Discipline of Oviedo, Florida feels is appropriate.

I hereby agree there will be no credits given to my self/child/ward in the video, audio, photograph or other media, or in any subsidiary works utilizing them in whole or in part. I hereby agree there will be no payments given to my self/child/ward for appearing in the video, audio, photograph or other media, or in any subsidiary works utilizing them in whole or in part. I hereby agree, on my own behalf and/or on behalf of my child/ward, to waive any rights or interest in and to the video, audio, photograph or other media, and will have no claim of any kind whatsoever against Conscious Discipline, its executors, licensees, successors or assignees based on the exercise of any rights granted hereunder or the use of the video, audio, photograph or other media.

I further agree, on behalf of myself and/or my child/ward, to waive the right to inspect and/or approve the products of Conscious Discipline that utilize the video, audio, photograph or other media, including any written copy that may be created in connection therewith. I hereby warrant I am of full legal age, have full legal authority, and am otherwise competent to sign this Release, and further warrant I have read it, understand it, and am fully aware of its contents.
